Etomidate and ketamine are the main induction agents for rapid sequence intubation (RSI) in emergency and critical care settings due to their relatively favorable hemodynamic profiles. Traditionally, etomidate has been preferred for its rapid onset and stability, despite concerns over transient adrenal suppression. On the other hand, ketamine has gained popularity for its sympathomimetic effects and preservation of adrenal function, though its direct myocardial depressant effects in catecholamine-depleted patients is worrisome.
