BACKGROUND:
Proper airway management remains vital for safe anaesthetic practice, contributing significantly to prevention of avoidable perioperative morbidity and mortality. The Difficult Airway Society (DAS) has periodically updated its guidelines to reflect evolving evidence, human factors, and technological advancements. The 2025 DAS Guidelines introduce strategies aimed at improving first-pass success, minimizing complications, and enhancing team-based responses to airway

OBJECTIVES:
To review the major updates in the 2025 DAS Guidelines and explore their practical implications for airway management across elective and emergency settings.

METHODS:
A narrative review of the 2025 DAS Guidelines will be presented, focusing on key domains including pre-assessment, planning, airway device selection, oxygenation strategies, and escalation pathways. Emphasis has been placed on changes from previous DAS recommendations and integration into clinical practice.

RESULTS:
The 2025 guidelines reinforce the importance of thorough airway assessment and structured planning, including the use of standardized cognitive aids. There is increased emphasis on first-pass success through routine use of video-laryngoscope, optimized patient positioning and apnoeic oxygenation. Awake tracheal intubation is further supported in anticipated difficult airways, with clearer indications and preparation protocols. The guidelines highlight early recognition of failed airway attempts, limiting repeated laryngoscopy, and prompt progression to supraglottic airway devices or front-of-neck access (FONA). Human factors—including communication, role allocation, and situational awareness, have been strongly emphasized. The guidelines incorporate advances in airway devices and advocate for regular simulation-based training.

CONCLUSIONS:
The 2025 DAS Guidelines represent a significant evolution toward safer, more standardized airway management. By prioritizing preparation, early decision-making, and team dynamics, these recommendations aim to reduce airway-related complications. Successful implementation will depend on clinician training, institutional support, and integration into local protocols.
